At the same time, there is an increase in the absolute number of leukocytes in the vascular bed (absolute, or true, neutrophilic leukocytosis). In a number of conditions, despite the increase in the number of neutrophils in 1 μl of blood, their absolute content in the vascular bed is invariable. Such relative neutrophilic leukocytosis is caused by the redistribution of leukocytes in the vascular bed with the transfer of a significant number of them from the wall (marginal) pool to the circulating pool. Extremely rarely, leukocytosis may be associated with a slower rate of excretion of leukocytes from the vascular bed (post-splenectomy leukocytosis).In a number of states, leukocytosis is caused by a combination of several pathogenetic mechanisms.
Eosinophilic leukocytosis is caused by the acceleration of the production and (or) release of eosinophils from the bone marrow into the blood (see Eosinophilia). Frequent causes of eosinophilic leukocytosis are immediate-type allergic reactions, in particular to drugs and vaccines.
Basophilic leukocytosis due to an increase in basophil production is a relatively rare hematological sign. An increase in the number of basophils in the blood can be observed during myxedema, ulcerative colitis, allergic reactions, during pregnancy. In these cases, the number of basophils increases slightly and does not lead to a significant increase in the number of leukocytes. Often the content of basophils increases with chronic myeloid leukemia, which is a prognostic unfavorable sign.
Lymphocytic leukocytosis (lymphocytosis) occurs in some acute (whooping cough, viral hepatitis) and chronic infections (tuberculosis, syphilis, brucellosis), and in infectious mononucleosis.Persistent lymphocytic leukocytosis is a characteristic symptom of chronic lymphocytic leukemia. Infectious lymphocytosis occurs with a pronounced increase in the number of lymphocytes in the blood, its mechanisms have not been finally clarified. In lymphocytic leukocytosis, the absolute number of lymphocytes in the blood increases (absolute lymphocytosis), which is caused by an increase in the flow of lymphocytes from the organs of lymphocytopoiesis into the blood.
Monocytic leukocytosis (monocytosis) is rare. Observed with bacterial infections (eg, tuberculosis, brucellosis, subacute septic endocarditis), as well as diseases caused by rickettsia and protozoa (malaria, typhus, leishmaniasis), malignant tumors (ovarian cancer, breast cancer, etc.), sarcoidosis, diffuse connective tissue diseases. The absolute number of monocytes in the blood is increased in patients with infectious mononucleosis, as well as in patients with agranulocytosis in the phase of the beginning recovery; a steady increase in the blood levels of monocytes is characteristic of chronic myelomonocytic and monocytic leukemias.An increase in the number of monocytes in agranulocytosis (indicates the beginning of the regeneration of blood formation) and in myelomonoblastic acute leukemia (indicating refractoriness to treatment) is of prognostic value.
